Prior to getting our Marketing Team involved with an event, project or task, please complete the form below. This helps us stay organized and minimize back and forth questions so we can be as diligent as possible in our delivery. After submitting this form, you should receive an email confirmation. If you do not receive one, it may indicate an error with your submission and you should send an email to our marketing team and attempt re-submitting the form again. Please submit 3-4 weeks prior to the needed deadline for the best product. Turnaround time varies per project.

Complete this form to start a project with the Marketing Department. We want to provide quality products and top-notch customer service. Submitting this form allows us to keep track of projects. Important Disclaimers

Timing - Lead Time: This form must be filled out 20 business days before Event/Activity*. Anything less than 20 business days will need to be evaluated on a case by case basis to avoid compromising quality and possible errors in communications and/or marketing materials.
Required Approvals Anything that involves a monetary investment is subject to an Executive Director / VP and President's approval. Before engaging with the marketing team, please consult with higher management to consult with the President (Dr. Burkett) and Finance (Jose Zamot). They will need to approve the cost.
Availability Certain promotional items included in this form may not be available. Although we manage multiple vendors, products available through different vendors may change from time to time and/or may be discontinued and/or we may have elected as a company to no longer order a particular item.
